Students New to Sesser-Valier CUSD #196

In order to enroll a new student at Sesser-Valier CUSD #196, the parent/guardian enrolling the child must provide the following items:

• The child’s official birth certificate with birth number (not a hospital copy)
• The child’s Social Security Card/number
• Proof of Residency – Category 1 – One (1) document from this list must be presented
  • Most recent property tax bill and proof of payment
  • Signed and dated lease and proof of last month’s payment
• Proof of Residency – Category 2 – Two (2) documents from this list must be presented showing current address:       
  • Driver’s license
  • Vehicle registration
  • Voter registration card
  • Most recent cable TV, credit card bill, etc.
  • Current public aid card
  • Current homeowners/renters insurance policy and premium payment receipt
  • Most recent gas, electric, or water bill
• A picture ID of the parent/guardian  (i.e. A copy of the driver’s license)

 Any new student enrolling in the district must have a valid Illinois physical, including shot records.
